Document Changes for Applications

Our active and retiree membership continues to grow as witnessed in the vital statistics area of our newsletter. In the process of coordinating with Georgia Firefighter Standards & Training Council, we found data errors on member files that lead to the potential of inappropriate awarding of creditable service which would have resulted in erroneous distribution of pension benefits. It is becoming increasingly important that we take measures to protect the long-term benefits of participants and their beneficiaries. Therefore, in our continuing efforts to make sure that the information we collect is accurate and the benefits we award are appropriate, we will be requiring additional information to accompany membership applications as well as retirement applications. Effective January 1, 2008, the following documentation will be required:

Membership Application
Proof of Date of Birth

Retirement Application
Proof of age of member (all retirees)
Proof of age of spouse (Joint & Survivor)
Proof of Marriage (Joint & Survivor)
Separation notice for full-time and part-time

With regards to date of birth, member's age, and spouse's age a copy of a birth certificate, driver's license, passport or any state or government approved document reflecting date of birth would be sufficient. Note that as a copy, it must be legible. (Sometimes drivers' licenses do not copy well.)

In order to provide proof of marriage a copy of a marriage certificate would be necessary. A separation notice can consist of a "Department of Labor Separation Notice" or notification of termination from the fire department where you were employed.
